Sick babies: Bronchiolitis vs Croup

So this time last week I was sitting in the ER with Baby Sid and Mom at Legacy Emmanuel Hospital.  If you live in the Portland area and need to take your child to the ER, go to this one (unless of course you don’t have time to choose, then go to your closest one!)  They have their own children’s emergency entrance, that’s right, one that is separate from adults.  The staff is great and they know how to deal with kids, and parents.

Baby Sid (3 months old) hadn’t been feeling well and earlier in the day he cried and it sounded like a barking seal.  Then he was fine, for the most part.  Keep in mind this is a very easy-going baby, not a lot of crying, he is a happy kid.  I was settling in for his last feeding of the night (because he already sleeps 8 hours! at night) when it sounded like he was having a hard time breathing.

So I called my mom….’Mom can you hear him? should I call the Dr? He is having a hard time breathing’  “YES! call the On-Call-Dr and have them listen to him breath, then call me back”

While talking to the On-Call-Dr Baby Sid started to cough and she said to take him in, it sounds like croup and at that age you don’t take chances.  Then she said “have someone ride in the car with you to monitor his breathing.”

Now our oldest is asleep, so I left my husband home with him and drove the 1.8 miles to my mom’s house and picked her up.  She drove to the hospital and I sat with Baby Sid, who was still having a hard time breathing, but smiled most of the trip.  While the Dr said to monitor his breathing she did not tell me what to do if he stops! but luckily nothing like that happened.

Fast Forward to the front desk nurse checking him out.  She says to the duty nurse whom she is passing us to “he has stridor but it’s not too awful.”  To which I reply, “well, not awful to YOU!” in slightly good humor.

ERVISIT

Stridor is a squeaky sound that is made upon inhalation.  It is caused by an obstruction of the breathing pathway.  It can be a symptom of both bronchiolitis and croup because in both cases a portion of the breathing pathway is inflamed and narrowed.

His blood oxygen levels were normal but he was taking about twice as many breaths a minute as he should have been.  They cleaned out his nose with a vacuum, boy that was fun! and sent his snot to the lab.  This improved his breathing rate but he was still squeaking with each breath.

The Dr said his lungs sounded good when he listened with the stethoscope and gave him a liquid steroid to open up his bronchial tubes and reduce the inflammation.  Because his lungs sounded clear the Dr said it wasn’t croup but bronchiolitis caused from Sid’s nasal drip.

What is the difference between the two?

The main difference is where the inflammation occurs in the lungs.

The cause can also be different but certain viruses cause both; like RSV, adenovirus, and parainfluenza, to name a few.  There are other causes of each and other symptoms.  Treatment is usually the same, steroids to reduce the swelling, and anything else you can do for a cold.

With croup the inflammation is in the trachea, around the vocal cords.

Bronciolitis is swelling or mucus build up in the bronchioles.  These are the smallest airways in the lungs.

You should always consult your healthcare provider to determine the cause of your symptoms.

Coincidentally my nephew, who was in town for the holiday, went to the same ER the very next night as was diagnosed with croup…

They sent Baby Sid’s nose mucus to the lab and it came back negative for the viruses listed above.  His lungs are still clear and his breathing is just fine.  Although he is still coughing and has a runny/stuffy nose.  I think the nasal drip is causing him to throw up a lot more than normal causing him to lose about 3 ounces (since he is above the seventy-fifth percentile for weight I am not concerned).

Did he have croup like my nephew or did he have bronchiolitis?  The only way we could have really determined it would have been a chest x-ray, but that is pointless.  The treatment would have been the same, so no need to expose him to unnecessary radiation.

He did good for his first, and hopefully last, ER visit.

12 Days of Christmas Tradition & Menu Plan Monday 12/17

When I was little we would go to Breakfast with Santa.  Over the years the quality went down hill and we stopped going.  Last year I wanted to take Ian, but looked into it too late.

This year I made sure we got tickets.  We went to the McMenamins breakfast at the Crystal Ball Room.

Fri: Kielbasa Tortellini Alfredo with Easy Parmesan Knots

Sat: Left Overs

Sun: Maple Bourbon Pork Chops with Parmesan Rice

*After making the cauliflower I puree it and add it to the mashed potatoes.

My Curve Ball

Earlier this week I had the boys in the car (a ’97 4Runner) with me on our way to the Wonderful-Sitter’s house.  Sid in his infant car seat (clicked into the base) on the passenger side and Ian in his toddler car seat on the driver’s side.

I was headed south down a road and there was a white minivanish vehicle sitting at the stop sign on a side street.

The next thing I know I was jolted like a pinball on the passenger side.  I look over to see coffee flying everywhere and splattering on the windows, the seats and all over our stuff.

My car goes sailing across traffic to the east side of the street, where I might have hit the sidewalk, and I pull to a stop on the shoulder.

I jump out of the car and run around to Sid’s side to see his car seat tilted on its side, base and all.  It was still buckled in, and not completely sideways because Ian’s car seat stopped it.

I had to unbuckle it to get it upright again.  Ian asks “what happen?” with no fear or concern.  I assessed the kids looking for any injuries asking Ian over and over again if he was hurt.  “I fine mama, I feel good, I not hurt.”

Sid was crying and I tried to calm him, but didn’t want to take him out of the car seat.  It was cold and there was a slight drizzle, plus I wasn’t sure if he was hurt and didn’t want to do anymore damage.

I was furious!  I couldn’t believe someone was so careless!  That feeling didn’t last long though, it quickly turned to shear and utter relief.  Everyone was OK, no apparent injuries.  (the verdict is still out on me, I have a lot of neck and shoulder pain which is giving me bad headaches)

The other car was finally able to cross the street and come over to us.  I didn’t ask for an explanation about what happened.  I knew what happened, he pulled out as I was driving by (see above diagram).  His car hit the front corner on my passenger side, then scratched the back door and hooked on my rear wheel well.  It doesn’t matter why he pulled out, just that he did.

We exchanged information and I called in the claim.  I could tell he felt bad about what happened, it was a mistake.  A bad careless one maybe, but still just a mistake.  When I was on the phone with the insurance agent I would look over at him and I could see it on his face that he was mortified about hitting my car.  Right then and there, I forgave him.

When I looked at my car I thought it was driveable so I took Ian to Wonderful-Sitter’s house and took Sid to the urgent care clinic (all within a mile of the accident).  While Sid had no apparent injuries I just had to be sure.  I couldn’t just ask him if he was hurt.  When I was talking to the insurance agent Sid kept crying, and he threw up, so I wanted to get him looked at.  He got a clean bill of health, with the exception of some spots that might have turned to bruises (but they didn’t).  The boys had a follow-up appointment with their pediatrician the next day.

As it turned out, my car was not driveable.  I could tell something was wrong so my sister came to get Sid and me from the urgent care clinic.  My car was towed to an auto body shop and declared TOTALED!  Unfixable.

Now I have to car shop!  Ugh. I am not looking forward to this.  I hate looking for a new car.  I should be happy, maybe I can get a minivan now….

When I drive the boys now I have this anxiety that runs through me.  No matter how good of a driver I am, there are just some things beyond my control.  I fear for their safety, although it hasn’t made me super paranoid, just more cautious.

It is always scary when you get in to an accident, but it much worse when your kids are in the car with you.

Life Throws a Mean Curve Ball

No matter how we prepare for life, it has a way of surprising us. My surprise came this week. Which means I have had a whole bunch else to deal with and the series I wanted to write about has been put on hold. Hopefully I will be able to finish it, but I also have so much else to write.
